What is AFC?
The Adult Family Care (AFC) program is an Arc program funded by MassHealth that pays family or non-family members a monthly tax free stipend to care for people in a home setting who are age 16 and older and who cannot live safely on their own.
Registered nurses and case managers offer ongoing education, advocacy, and healthcare oversight through monthly home visits, phone consultations and 24/7 on-call emergency support. The goal of the program is to keep people living in a home setting.

Eligibility & Requirements
Caregiver Requirements:
- Must be 18 years of age
- Can be biologically related to the AFC
member, but a legal guardian or spouse - Must complete a comprehensive application process, including
background checks, annual physical, and home study - Provide 24/7 care* for the AFC member in a home setting in Massachusetts
- Accept responsibility for the person’s care and supervision of all health related activities.
Member Eligibility:
- MassHealth insurance
- At least 16 years of age
- Have a written order for AFC services from their Primary Care Physician
- Have a medical diagnosis, physical, intellectual or developmental disability and require supervision or assistance with at least one of the following: eating, bathing, dressing, toileting, ambulating, or transferring.

- Contact Heather Hess, Director of AFC, to schedule a consult to determine basic eligibility
- Schedule a home visit for assessment, evaluation, and training.
